The patient has a history of DLBCL-GCB s/p DA-R-EPOCH. Recent PET-CT showed interval progression of metabolically active disease involving the lymph nodes of the left supraclavicular region, mediastinum, and hila with development of a solitary focus in the left iliac bone.

Histologic sections show needle-shaped fragments of lymph node with architectural effacement by a diffuse neoplastic lymphoid infiltrate with extensive necrosis.
